During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
* Effective 12/09/2024, the Office of Background Investigations implemented changes to their provider portal. Please note that all providers will need to create an account to request Central Registry background clearances. Please contact OBI directly if you have questions or concerns. Here is a caption from the memo that was sent on 12/04/2024: Both current and new users will need to register a new account to access the new OBI Provider Portal after December 9, 2024, to ensure all features of the enhancements are applied to user accounts. Registration will create an account with up to 5 authorized users and will allow the primary account user to update account access and facility contact information as needed.

* Standard 8VAC20-780-340 D (for CDCs) requires a staff member that meets the qualifications of a program leader or program director to be `regularly present? in each grouping of children. As stated in the exception in 8VAC20-780-340 E, a program leader is not required in each grouping of children during the first and last hour of operation when a center operates more than six hours per day and during the designated rest period (nap time) if certain requirements are met. The standards do not provide additional exceptions or occurrences when it is acceptable for a program leader to not be present. However, it is reasonable to permit flexibility during short breaks (e.g., a restroom break).

* Discussion (for CDCs) that the Code of Virginia defines school age as five by September 30th (? 22.1-1). A child who will turn 5 by September 30 within the current year is considered school-age on the day following the last day of the school year of the school division in which the child resides, typically in May or June. A preschool class that operates in the summer months in which all of the children turn 5 by September 30 is not required to follow standards specific to preschool-aged children. For example, the ratio in the group can change from 1:10 to 1:18 regardless of whether the class is called ?preschool,? because all the children in the group are school-aged.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 02/28/2025. The on-site inspection began at 9 m and ended at 11 am. The inspector reviewed compliance in the areas listed above. There were (79children present and (12staff. The inspector reviewed (5 children?s record, and (2) staff records on-site on 02/28/2025.

This inspection included: document review (i.e. files, policies and procedures, emergency drill logs, medication, administration logs, authorization forms); tour of the facility; interviews; and observations.

Information gathered during the inspection determined a non-compliance with an applicable standard or law. This violation was documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-510-E
Description: Prescription medication may be administered if the parent has completed a written authorization from signed by a parent and physician.

The name of the medication to be administered to Child #1, did not reflect the name of the medication listed on the signed authorization form.

Plan of Correction: A corrected authorization form will be obtained and placed on file.
