During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Please revise the refrigerator plan for the infant room.
Complete Health Sanitation Inspection during your next VDH visit in June.
Update Child registration form to include the ?date viewed? for child?s birth certificate and child?s gender.

Comments:
An unannounced, on-site monitoring inspection was initiated on 2/25/2025 and completed 2/25/2025. The on-site inspection began at 10:03 am and ended at 12:56 pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and six staff. The inspector reviewed five children?s records and five staff records on-site on 2/25/2025.

This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/10/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-270-A
Description: REPEAT VIOLATION
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, rusting of any equipment, chipped or peeling paint; visible cracks,
bending or warping, rusting or breakage of any equipment.

In the preschool room, the sheets used to cover the cots during naptime were stained and the oversized pillows used on the floor for sitting had multiple stains.

In the Infant Classroom the following items were observed; the sheets on the cribs were stained; the covering on the infant bouncers were stained; the paint was chipped and peeling along the wall near the play kitchen area; the rug being used by infants during reading time had multiple stains and loose particles on the rug.


In all three classrooms, the floors had
an accumulation of dirt and stains along the wall borders and
corners of the rooms.

Plan of Correction: Areas and equipment would be maintained in a clean, safe and operable condition.
Preschool room sheets, covers and cots will be cleaned or discarded.
The infant room would be redone with paint and etc to maintain chipped walls and loose particles along wall perimeter.

The classrooms would be cleaned and free of dirt and stain in classrooms.
