During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site renewal inspection was initiated and completed on 02/27/2025. The on-site inspection began at 4:00 p.m. and ended at 5:15 p.m. The inspector reviewed compliance in the areas listed above. There were 25 children present and 5 staff. The inspector reviewed 8 children?s records and 5 staff records.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/10/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
Staff A, employed for more than 5 months, did not have documentation of requesting a central registry check or a sex offender registry check from the state the individual resided within the preceding five years.Plan of Correction: Facility Administration will request the required out of state central registry check and out of state sex offender check for staff A.
Standard #: 8VAC20-780-540-E Description: The facility's emergency supplies are required to have one working, battery-operated radio in the building that is used by children
A battery-operated radio was not available in the facility during the inspection.Plan of Correction: Administration will purchase a battery-operated radio to accompany the facility's emergency supplies.
