During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was conducted on 3/3/2025. The on-site inspection began at 10:09 am and ended at 2:25 pm. The inspector reviewed compliance in the areas listed above. There were 100 children present and 16 staff. The inspector reviewed 10 children?s records and 10 staff records on site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/11/2025.
A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-B-2
Description: Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Employee #2's national criminal background check was completed 3 days after the start of employment.

Plan of Correction: double check and confirm that fingerprints have been received. Do not have teachers train.

Standard #: 8VAC20-780-160-A
Description: Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children.

Staff #2, who had been employed for 2 weeks, did not have a completed TB on file. Administration stated that the TB screening had not been submitted to the center.

Plan of Correction: Do not let teachers start before they bring in the written negative TB test.

Standard #: 8VAC20-780-60-A-8
Description: A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #7 and child #9, who have been enrolled for six months, have a diagnosed food allergy and did not have a written care plan in their file.

Plan of Correction: re-submit allergy plan to parent with a deadline. Once received, it will be added to their file.

Standard #: 8VAC20-780-280-B
Description: Hazardous substances must be in a locked place that prevents access by children.

In the younger infant classroom, a cannister of disinfectant wipes was in an unlocked cabinet, out of the reach of children. In a two year old classroom, a bottle of glass cleaner was unlocked on top of a shelf, out of the reach of children. In a second two year old classroom, an unlocked cabinet contained three cannisters of disinfectant wipes, glass cleaner, and sanitizer. The cabinet was not within the reach of children. In a three year old classroom, a container of bleach was in an unlocked cabinet within the reach of children. The bleach contained warning labels to include, "keep of out of reach of children."

Plan of Correction: communicate with teachers about locking those items away. During am/pm check manager will confirm that teachers are in compliance and any class not in compliance will receive the proper coaching form.
