During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Technical Assistance provided in the area of mandated reporting. As mandated reporters, the center is reminded to report any and all suspicions of child abuse or neglect to the local Child Protective Services (CPS) hotline, and allow the CPS office to make the determination as to whether or not to move forward with an investigation.

Comments:
An unannounced inspection was initiated on 02/24/2025 and completed on 02/27/2025, with a local agency in response to a complaint received by the licensing office on 02/24/2025 relating to allegations of care and protection. The inspector reviewed 1 children?s records and 1 staff records on-site on 02/27/2025. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/13/2025. A POC submitted after this date will not appear on the public website.

Description: Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On 2/19/2025, a video recording provided by the center observed that staff #1 did not provide age and stage appropriate redirection to a child that had thrown an object and pushed another child.

Plan of Correction: Staff #1 is separated from the company. Remaining Center staff have been retrained on appropriate redirection as well as prohibited actions. Center administration will continue to monitor staff for complaince.

Description: The following actions or threats thereof are forbidden: roughly handling a child.

On 02/19/2025, a video recording provided by the center showed staff #1 grabbed the hand of child #1. Staff #1 pulled their arm, and subsequently picked the child up by one arm, holding their upper arm, before then grabbing the other arm to carry them to a nearby chair. Child #1 was then sat into a chair, staff #1 held their head back while talking to them. Child #1 then got up from their chair, while staff #1 spoke with a parent. Once the parent left, staff #1 then grabbed child #1 again by the upper arms to roughly sit them back in the chair.

Plan of Correction: Immediately upon notification of the incident, Staff #1 was excluded from attending work and is now separated from the company.
