22.1 BACKGROUND CHECKS, CODE, CARBON MONOXIDE

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 02/25/2025. The Department issued a Notice of Intent to revoke the license on 02/19/2025 as a result of high-risk violations. The on-site inspection began at 9:35 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and 5 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-E
Description: REPEAT VIOLATION SYSTEMIC VIOLATION

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

Child 2, who has been attending for more than two years, did not have documentation of obtaining of additional immunizations every six months until the child turned 2 years old.

Plan of Correction: Contacted the child's mother during the inspection; the child cannot return until the information is provided.

Standard #: 8VAC20-780-140-A
Description: SYSTEMIC VIOLATION

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

Child 2, who has been attending for more than two years, did not have a physical exam on record.

Plan of Correction: Contacted the child's mother during the inspection; the child cannot return until the information is provided.

Standard #: 8VAC20-780-160-C
Description: SYSTEMIC VIOLATION

A tuberculosis (TB) screening is required every two years from the last screening.

Staff 3?s updated TB screening was 10 months past due.

Plan of Correction: Texted the staff during the inspection requesting an updated screening.

Description: REPEAT VIOLATION

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

There were areas of broken drywall in both the men and women?s restroom, which exposed the insulation inside of the wall; the insulation was accessible to the children. There were areas of rust on the stall walls in both the men?s and women?s restrooms. The wood at the bottom of the wooden cover around the sink of Classroom 1 was splintered and sharp; the splintered and sharp edges were accessible to the children.

Plan of Correction: Has been brought to the church board and will be fixed.

Description: REPEAT VIOLATION

Children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

The children in Classroom 3 did not wash their hands after eating lunch.

Plan of Correction: Will retrain the staff.

Standard #: 8VAC20-780-560-M
Description: No child shall be allowed to eat or drink while walking around.

A child was observed walking around Classroom 3 while eating food from their lunch.

Plan of Correction: Will retrain the staff.
