Comments:
An unannounced, on-site monitoring inspection was initiated and completed on March 6, 2025 as a part of the licensure period. The on-site inspection began at 10:08amand ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 42 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-50-A
Description: Children's records shall be treated confidentially.

In Classroom A, a list of children's names with allergies and sensitivities was posted without a cover sheet.

Plan of Correction: The list was removed from the wall at the time of the inspection. Allergy lists are covered in each classroom.

Standard #: 8VAC20-780-70
Description: Required records were not kept for each staff person.

Staff #4 did not have documentation to demonstrate that that they possessed the education, certification, and experience required by the job description, orientation and training as required. Staff #4 is identified as the lead teacher of Classroom A and has been employed for six months.

Plan of Correction: Staff #4 will provide a copy of their proof of education to management to be placed in the record.

Standard #: 8VAC20-780-550-D
Description: The center shall implement a monthly practice evacuation drill.

The center did not practice a monthly practice evacuation drill for the months of December and January.

Plan of Correction: Moving forward, the center will ensure that monthly evacuation drills are conducted. The drills for the months of December and January were not conducted due to weather conditions and holidays.
