An unannounced inspection was initiated on 2/26/2025 and completed on 3/10/2025 in response to a complaint report received by the licensing office on 2/24/2025 relating to allegations of administration, medication administration, and special care. The inspector reviewed 1 child?s record on-site. This inspection included document review, a tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, this complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, other violations were found related to administration, special care provisions and programs are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-90-A Complaint related: No Description: A written agreement between the parent and the center shall be in each child's record by the
first day of the child's attendance. The agreement shall be signed by the parent and include an authorization for emergency medical care.
Child #1s record did not contain a written agreement signed by the parent for emergency
medical care.Plan of Correction: Prior to enrollment, all staff will ensure paperwork is completed and correct. Documentation will be reviewed prior to child's first day of attendance.
Standard #: 8VAC20-780-440-J Complaint related: No Description: There shall be at least twelve inches of space between the sides and ends of occupied cribs
and thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib.
Classrooms #2 and #3 did not have at least twelve inches of space between the sides and
ends of occupied cribs or thirty inches of space between service sides of occupied cribs and
other furniture.Plan of Correction: During the inspection, cribs were moved to ensure 12 inch and 30 inch spacing. When children are in cribs, staff measure for accurate spacing.
Standard #: 8VAC20-780-500-A Complaint related: No Description: Staff shall wash their hands with soap and running water before and after a diaper change. Children's hands shall be washed with soap and running water after toileting.
Staff #5 did not wash their hands or the child?s hands after diaper change.Plan of Correction: All staff to be retrained immediately on diapering and toileting protocols. A sheet was created and posted in classrooms that details all required procedures.
Standard #: 8VAC20-780-500-B Complaint related: No Description: The diapering surface shall be cleaned with soap and at least room temperature water and
sanitized after each use. Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Staff#5 did not clean or sanitize diapering surface after it was used and used their hands to dispose of a soiled diaper, instead of foot pedal.Plan of Correction: All staff to be retrained immediately on diapering and toileting protocols. Medication shall be kept in a locked place using a safe locking method that prevents access by children.
In Classroom #2, a child?s prescription medication was not stored locked.Plan of Correction: A designated medication storage location for all medications that is locked has been assigned.

Centers shall keep a record of medication given children, which shall Include the day and time the medication was administered to the child and any medication error.
Medication administration log indicated medication was given to child at 12:00pm, however it was not given at the documented time and error of time was not documented.Plan of Correction: Staff will ensure that medication administration is logged accurately with time medication is given.
Standard #: 8VAC20-780-570-A Complaint related: No Description: When a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.
In Classroom #1, there were no protective belts on high chairs being used by 3 children.Plan of Correction: Safety belts were replaced.
