Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site renewal inspection was initiated on 3/3/2025 and completed on 3/3/2025. The on-site inspection began at 12:30pm and ended at 2:50pm. The inspector reviewed compliance in the areas listed above. There were twenty-one children present and four staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/18/2025. A POC submitted after this date will not appear on the public website.

Description:	The center is required to have employees undergo a background check every five years. The most recent central registry check for Staff #2 was overdue by more than nine months.

Description:	The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #2, did not have a copy of their immunizations until 11 days after start date.

Description:	Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. 1. A bottle of Draino, 2 containers of Clorox wipes, a spray bottle of Windex, and a container of Lysol toilet bowl cleaner with warning labels, ?Keep out of reach of children,? were unlocked in the boy's bathroom cabinet, within reach of children. 2. A container of Clorox wipes with warning labels, ?Keep out of reach of children,? were found in the kitchen area, out of reach of children. 3. Description:	Cots used by children during a rest period shall have linens consisting of a top cover and a bottom cover. Two of the nine cots in the Pre-K room did not have a bottom cover on the rest mats.

Description:	Bottles shall not be propped or used while the child is in his designated sleeping location. A child over the age of one was found asleep in a crib with a bottle full of milk.
