During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated and completed on 03/04/2025. The on-site inspection began at 11:00 am and ended at 1:06 pm. The inspector reviewed compliance in the areas listed above. There were 19 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/18/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-A Description: The center is required to have employees undergo a background check every five years.
1. The most recent central registry check for staff #3 was overdue by 4 months.
2. The most recent central registry check for staff #4 was overdue by 6 weeks.
3. The most recent criminal background check for staff #3 was overdue by 5 months.Plan of Correction: 1. The staff that needed the renewal for her Central Registry Check completed all forms on the day of the inspection. Record request was submitted on the same day and "clearance" results received on 3-7-25. This staff has been continuously employed at River's Bend Children's Center for over 5 years.
2. The staff that needed the renewal for her Central Registry Check completed all forms on the day of the inspection. Record check request was submitted on the same day and "clearance" results received on 3-7-25. This staff has been continuously employed at River's Bend Children's Center for over 11 years.
3. The staff that needed the criminal background check renewal had fingerprint data collected on 3-6-25 and "clearance" results returned on 3-10-25. This staff has been continuously employed at River's Bend Children's Center for over 5 years.
Standard #: 8VAC20-780-40-M Description: The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present.
Child #1, enrolled for 4 months, has a documented dietary restriction. Child #1 was not listed on the posted allergy list in the classroom dated 03/03/2025.Plan of Correction: Child #1 had a dietary restriction. Although it was not included on the classroom allergy list, all staff were immediately informed of the restriction. Staff abided by this restriction and never once served the child the dietary restriction. The child's food intolerance was added to the posted list the same day of the inspection. By current state licensing regulation, the "posted allergy list" is not permitted to be posted in plain sight. As 8VAC20-780-40 (M) states the following : This list shall be dated and kept confidential...Centers typically maintain this confidentiality mandate by placing a cover sheet over the allergy list. We currently use this method. Up until October of 2021, child care centers were permitted to display allergy and/or food intolerances of children in plain sight of staff. The Standards for Licensed Child Day Centers is currently undergoing a revision and if the revised standards are adopted, centers will be permitted to openly display food allergies to staff with written permission from the child's parent. We believe this is the best practice as this information will always be easily within sight.
