An unannounced, on-site renewal inspection was initiated on 03/04/2025 and completed on 03/10/2025. The on-site inspection began at 10:30 am and ended at 2:40 pm. The inspector reviewed compliance in the areas listed above. There were 57 children present and 13 staff. The inspector reviewed 5 children?s records, 5 staff records, and 4 applicant records.
The inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: The center is required to have employees undergo a background check every five years.
The most recent criminal history record check for Staff 4, was over by 18 months.Plan of Correction: The staff member had the fingerprint check conducted on Friday.
Standard #: 22.1-289.035-B-4 Description: REPEAT VIOLATION
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
The record for Staff 1, employed for 1 month, contained information that to indicate that they have resided in another state. The record did not contain documentation of requesting or receiving a criminal history record check or a child abuse and neglect registry check from that state.Plan of Correction: Will request the information.
Standard #: 8VAC20-780-140-A Description: REPEAT VIOLATION
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
The record for Child 1, who has been enrolled for 2 months, did not contain a physical exam. The record for Child 2, who has been enrolled for 6 months, did not contain a physical exam.Plan of Correction: Have been requested. Moving forward, physical exams will be required before first day of attendance.
Standard #: 8VAC20-780-160-C Description: REPEAT VIOLATION
A tuberculosis (TB) screening is required every two years from the date of the last screening.
The updated TB screening for Staff 4 was obtained 5 months past the required timeframe.Plan of Correction: A system will be created to track dates to ensure they are obtained within the required timeframe.
Standard #: 8VAC20-780-270-A Description: The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
The wire on the bunny cage in Classroom 2 had multiple sections that were sharp and could snag skin; these sharp sections were accessible to the children in the class. The door on the wooden play stove in Classroom 2 was broken, creating a pinch point that was accessible to the children.Plan of Correction: The stove has been removed. Will attempt to make the repairs on the bunny cage or will come up with a new cage option.
Standard #: 8VAC20-780-500-A Description: Children?s hands shall be washed with soap and running water or a disposable wipe before and after eating meals and snacks.
Child 4 was observed returning to a play area immediately after finishing the morning snack; Child 4 did not wash their hands with soap and running water or a disposable wipe after eating the snack.Plan of Correction: Hand wipes will be added to the class.
Standard #: 8VAC20-780-560-F Description: When centers choose to provide meal or snacks, a menu listing the foods to be served for meals and snacks during the current one-week period shall be dated; posted in a location conspicuous to parents; list any substituted food; and be kept on file for one week at the center.
The center does not provide a current one-week menu to families. Items served are communicated to families on a daily basis through a communication app used by the center.Plan of Correction: All classrooms now have posted menus and they will be kept on-file for one week.
