Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced inspection was initiated and completed on 3-3-25 in response to a complaint received by the licensing office on 2-26-25 relating to allegations of inadequate supervision. The inspector reviewed 1 staff record on-site on 3-3-25. This inspection included document review, tour of the facility, interviews, observations, and measurements. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/19/25. A POC submitted after this date will not appear on the public website.

Description:	(Repeat Violation)For staff hired prior to July 1, the center is required to ensure that staff obtain documentation that a central registry search is completed by the end of the 30th day of employment. Evidence: The record for Staff #1 did not contain documentation that a central registry search was completed within 30 days of employment. There were no Central registry results in the file. No documentation of follow up was in the file.

Description:	The Center is required to ensure that all areas and equipment of the center, inside and outside, are maintained in a clean, safe, and operable condition. Evidence: In the Creme TV room, the back wall next to the windows, has a 24-inch area of paint chipping from the wall.
Plan of Correction:	Wall is being repainted. Classroom is closed until then.

Description:	(Repeat Violation) The Center is required to ensure that each grouping of children has at least one staff member that meets the qualifications of program leader or program director regularly present. Evidence: Staff #1 does not have documentation on file that they meet the qualifications of a program leader or program director.

Description:	(Repeat Violation) The Center is required to ensure that children under 10 years of age always be within actual sight and sound supervision of staff. Evidence: 1)On 2/25/25, Staff #1 was observed in the Creme TV classroom alone with 3 children (age 4) and was found sleeping with their head on their arm on the desk in the back of the classroom. 2)Staff #1 did not awaken or look up when 2 different people entered the classroom, and was aroused from sleep once they were spoken to by another staff member.

Description:	(Repeat Violation) The Center is required to ensure they have at least one staff member in each classroom or area where children are present that has obtained first aid and cardiopulmonary resuscitation (CPR) certification with an in-person competency demonstration. Evidence: The file for Staff #1, who was alone with children on 2/25/25, does not contain documentation for completion of first aid and cardiopulmonary resuscitation (CPR) certification.
Plan of Correction:	All staff members have received updated CPR training.
