An unannounced inspection was initiated on 2-7-2025 and completed on 3-12-2025, in response to a complaint received by the licensing office on 2-5-2025 relating to allegations of Special Services, Staffing /Supervision and Programs. The inspector reviewed 1 child's records and
5 staff records on-site. This inspection included document review, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3-19-2025.

Standard #: 8VAC20-780-340-A Complaint related: Yes Description: Repeat Violation
The center failed to ensure when staff are supervising children, they shall always ensure their care, protection, and guidance. Center staff confirmed that on 2-3-2025, Child #1 was fed a mixture of bananas and breast milk that belonged to Child #2.Plan of Correction: 2/6/2025: The employee that verified the bottle is no longer employed with A+ Childcare.
2/4/2025:Each child has a designated bin in the refrigerator for their bottles and meals. Each bin is labeled with the child?s first and last name, along with a picture to clearly identify the owner of the contents.
2/10/2025: Each employee will complete infant feeding training with a member of the administration. This training will include a demonstration of the bottle verification procedures as well as location of feeding schedules.
2/10/2025: The opening teacher will take a photo of all bottles and meals brought in by the parent immediately upon drop-off. This photo will be saved in our app, allowing staff to easily access it for an additional layer of verification.
3/1/2025: We have reduced the number of teachers in the classroom to only 4 teachers.
Standard #: 8VAC20-780-340-D Complaint related: Yes Description: The center did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Such a program leader shall supervise no more than two aides. On 2-3-2025, in Room #1, Staff #3, a program leader, was supervising 3 aides, Staff #2, Staff #4 and Staff #5. Staff #2 had been working for less than 1 week; Staff #4 had been working for less than 1 month and Staff #5 had been working for 1 day. Plan of Correction: We have developed a training plan for all new staff that includes specific dates and times for hands-on training with a program lead. This approach allows administration to stagger new hires, ensuring that the program lead trains only one new staff member at a time.
