During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated on February 27, 2025 and completed on February 27, 2025. The on-site inspection began at 11:39 a.m. and ended at 2:30 p.m. The inspector reviewed compliance in the areas listed above. There were 61 children present and 17 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included record review, tour of facility, observations, and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 19, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-210-A Description: Program leaders shall be at least 18 years of age, have fulfilled a high school program completion or the equivalent, and meet one of the required qualifications.
Staff #1, Staff #4 and Staff #5 did not complete all the required training to be lead teacher.
Staff #2's record does not demonstrate how he/she meets lead qualifications. All four staff records list the staff as lead. Staff #1 and Staff #2 were acting as lead in a classroom the day of the inspection.Plan of Correction: 1. Playground Safety training will be done by Staff #4 and #5 making them qualified leads.
2. In the future the director will ensure proper documentation clearly states how staff meet lead qualifications.
Standard #: 8VAC20-780-570-E Description: Prepared infant formula shall be refrigerated, dated and labeled with the child's name.
Two bottles in the infant room were not dated and labeled with the child's name.Plan of Correction: 1. Staff are to check bottles and food for name and date labels upon arrival of child and remind family of labeling policy if needed.
2. Staff to label when incompleted by family.
