Comments:
An unannounced inspection was initiated and completed on 02/25/2025 in response to a complaint received by the licensing office on 02/22/2025 relating to allegations of Staffing and Supervision, Special Care Provisions and Special Services. The inspector reviewed 2 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation of staffing and supervision, therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Description: The required ratio for children ages 16 to 24 month is one staff per five children.
On 02/25/2025, between 8:15a.m. to 8:30a.m. a total of 7 children with only 1 staff was present in a classroom.

Plan of Correction: 1. Immediate Staffing Adjustment: February 25, 2025

? Effective immediately, all classrooms will be monitored to ensure compliance with the required staff-to-child ratios. Ratio by Ms. Veronica is taken every hour in hours of center operation and from 3pm to close it is taken every 15 mins.

? A floating staff member will be assigned during peak hours to provide additional coverage if necessary.

2. Staff Scheduling & Coverage:

? The facility will review and adjust staffing schedules to ensure appropriate coverage during transitions, breaks, and peak hours.

? A backup staff list will be created to ensure immediate availability in case of staff absences.

3. Training & Compliance Monitoring:

? All staff will receive mandatory refresher training on ratio compliance by March 20, 2025.

? Classroom ratios will be monitored daily by the center director and lead teachers.

? Weekly unannounced spot-checks will be conducted by the management team for the next 60 days to ensure compliance.

4. Documentation & Accountability:

? A daily ratio log will be implemented where teachers sign in and out, tracking the number of children and staff present at any given time.

? Any instance of non-compliance will be documented, and corrective action will be taken immediately.

Description: A menu listing foods to be served for meals and snacks during the current one-week period shall list any substituted food.
The menu for 02/25/2025 included whole wheat English muffins for breakfast. White English muffins were served. This substitution was not listed on the current menu.

Plan of Correction: 1. Menu Compliance & Communication:

? Effective immediately, any food substitutions will be recorded on the posted menu before meals are served.
? A designated staff member (e.g., kitchen manager or lead teacher) will be responsible for updating the menu-board daily to reflect any changes.

2. Training for Food Service Staff & Teachers:
? Kitchen and classroom staff will receive training on proper menu documentation and compliance with food substitution regulations by March 20, 2025.

3. Monitoring & Record-Keeping:
? The center director will review menus weekly to ensure accuracy and compliance.

? A Food Substitution Log will be introduced to track all changes, including reasons for substitution and staff responsible for updating the menu.
4. Parental Communication:

? Parents were notified of any major or menu substitutions through the facility's communication email stated in a letter, and daily meal reports.
