During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site focused monitoring inspection was initiated and completed on 3-3-25. The on-site inspection began at 1:55pm and ended at 4:50pm. The inspector reviewed compliance in the areas listed above. There were 46 children present and 17 staff. The inspector reviewed 2 children?s records and 4 staff records on-site on 3-3-25. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3-21-25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-B-3
Description: (Repeat Violation) For staff hired prior to July 1, the center is required to ensure that staff obtain documentation that a central registry search is completed by the end of the 30th day of employment.

Evidence: The record for Staff #1 and Staff #2 did not contain documentation that a central registry search was completed within 30 days of employment. There were no Central registry results in the file. No documentation of follow up was in the file.

Plan of Correction: Central registry check has been submitted. Staff is not working until check is returned.

Standard #: 22.1-289.035-B-4
Description: (Repeat Violation) The Center is required to obtain the results of a central registry check from all states in which staff members have resided within the last five years prior to employment.

Evidence:
The record for Staff #3 did not contain documentation of a completed central registry check from 2 states where they resided within the last five years prior to employment.

Plan of Correction: Out of state check for staff member has been completed.

Standard #: 8VAC20-780-160-A
Description: (Repeat Violation) The Center is required to obtain documentation of a negative tuberculosis (TB) test or screening for staff at the time of employment and prior to contact with children and within 30 days prior to employment.

Evidence:
The file for Staff #1 and Staff #4 did not contain documentation of a negative tuberculosis (TB) test or screening.

Plan of Correction: Staff have obtained TB tests.

Standard #: 8VAC20-780-340-F
Description: (Repeat Violation) The Center is required to ensure that children under 10 years of age always be within actual sight and sound supervision of staff.

Evidence: During inspection in the Infant A room, there were 2 staff members sitting on the floor with 2 infants. On the other side of the half wall located within the Infant A classroom, there were 2 infants (ages 10 months and 14 months) sleeping in cribs. Neither staff member could see either of the infants in the cribs from where they were seated.

Plan of Correction: Infant team has been retrained on how to maintain sight and sound at all times.
