During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. This inspection report was amended on March 20, 2025.
An unannounced, on-site monitoring inspection was initiated on March 10, 2025, and completed on March 12, 2025, as a part of the licensure period. The on-site inspection began at 2:58 pm and ended at 4:45 pm. The inspector reviewed compliance in the areas listed above. There were 32 children present and three staff. The inspector reviewed five children?s records and three staff records electronically on March 12, 2025. This inspection included document review tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 21, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-70 Description: A staff record shall be kept for each staff person that contains documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, and orientation and training as required. The record of staff #1 and staff #2, both identified as program leads, did not contain documentation that the individuals possessed the education and experience required by the job position. Standard #: 8VAC20-780-510-P Description: When an authorization for medication expires, the parent shall be notified that the medication
needs to be picked up within 14 days or the parent must renew the authorization.
Medications that are not picked up by the parent within 14 days will be disposed of by the
center by either dissolving the medication down the sink or flushing it down the toilet.
Medication for child #4 was onsite, however the medication authorization had expired six
months ago. The medication had not been administered past the expiration of the
authorization. The authorization had not been renewed, and the medication was not returned or disposed of as required.Plan of Correction: Not available online. Contact Inspector for more information.
