Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site renewal inspection was initiated on March 17, 2025 and completed on March 17, 2025. The on-site inspection began at 1:10pm and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There were 37 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records [on-site]. This inspection included: document review, tour of the facility, interviews, and observations. and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 25, 2025. A POC submitted after this date will not appear on the public website.

Description:	Prior to hire staff must provide a sworn statement or affirmation disclosing whether he has ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth. Staff 1's record did not contain a sworn statement or affirmation.

Description:	The center must follow their own policies and procedures that are required by these standards, and the terms of the current license issued by the department. The center did not follow it's safe sleep policy. Four infant were observed sleeping in bouncy chairs.

Description:	During the stated hours of operation, there always shall be on the premises one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. At the beginning of the inspection all staff members were caring for children and counted in ratio. There was not an additional staff member immediately available. Staff 4 arrived at the center from picking up school age children from school approximately twenty minutes after the start of th inspection.

Description:	REPEAT VIOLATION In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. At the beginning of the inspection Staff1 was the sole care provider in Classroom A. Staff 1 does not meet the qualifications of a program leader.

Description:	REPEAT VIOLATION The center shall maintain a ratio of one staff for every four infants under the age of sixteen months. At the beginning of the inspection the Classroom A had a ratio of one staff for seven infants. A second teacher arrived to the classroom after approximately ten minutes.
