During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated and completed on 3/17/25. The on-site inspection began at 10:05am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 90 children present and 16 staff. The inspector reviewed six children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews,
and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/26/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-130-A
Description: (Repeat Violation) The center is required to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The record for Child K, who had been enrolled for more than 20 months, did not contain documentation of immunizations.

Plan of Correction: Immunizations located in another file after visit.

Standard #: 8VAC20-780-270-A
Description: Areas and equipment of the center, inside and outside, are required to be maintained in a safe condition.

On the small playground there were concrete pathways going throughout the mulched area. The mulch had washed away or condensed down to create tripping hazards where the mulch did not meet the edge of the concrete pathways. A child was observed tripping, falling, and crying during the inspection when they ran from the mulched area and caught their shoe on the side of the concrete path.

Plan of Correction: Mulch delivered the day after inspection.

Standard #: 8VAC20-780-330-B
Description: (Repeat Violation) Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child?s trajectory in the event of a fall. The composite play structure on the large playground required six feet of resilient surfacing on all sides, that is at least six inches deep for wood mulch.

At the bottom of each slide on the large playground the wood mulch was breaking down and measured less than two inches deep.

Standard #: 8VAC20-780-510-E
Description: (Repeat Violation) Long-term prescription drug use and over-the-counter medication require written authorization from the child?s physician and parent.

The medication authorization forms containing physician?s authorization for long-term medication were expired for Child C, Child E, and Child F. The form contained documentation stating that it expired within one year of signature. An updated form was overdue for Child C by more than 14 months. An updated form was overdue for Child E by more than nine months. An updated form was overdue for Child F by more than six months.
