Two licensing inspectors conducted an unannounced, on-site focused monitoring inspection which was initiated and completed on March 19, 2025. The Department issued a Notice of Intent to Deny Renewal Application for a License on December 10, 2024, as a result of high-risk violations. The inspector reviewed compliance in the areas listed above. There were 108 children present and 21 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 27, 2025. A POC submitted after this date will not appear on the public website.

Systemic Violation
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
1. Visible dirt/debris and a cobweb were found on the floors and along baseboards in the downstairs classrooms and hallways. Pieces and crumbs of orange cheddar chips served the previous day were under all six tables and on the round carpet in the school-age room.
2. The downstairs children?s bathrooms contained toilet paper on the floor, visible dirt shoeprints and dirt around the toilet.
3. A child in the 2-year-old class took the salad tongs from the counter and played with them and then placed them on the children?s table. Staff #1 then rinsed the tongs off with water and used them to serve food.
4. There were several cracked and broken floor tiles throughout the center.Plan of Correction: Reviewed cleaning checklist w/staff bathrooms + classroom will be checked through the day to ensure cleanliness

Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. There were 14 gallons of wall paint and a gallon of floor coating in an unlocked storage room in the Pre-K 1 room (ages 3-4).Plan of Correction: Knob removed from classroom access
