Comments:
-This annual subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic.
Preliminary data was obtained from the vendor on September 25, 2020.
-An annual subsidy inspection was initiated on September 25, 2020, at which time the vendor was contacted by video call to initiate the inspection, and concluded on that same day. The inspector emailed the vendor a list of items required to complete the inspection.
The vendor reported there were 3 children in care but not present on the time of the inspection and 1 children living in the home under the age of 12 present with the vendor. The inspector reviewed 3 children?s records and 1 adult/household member record submitted by the vendor to ensure compliance with the health and safety regulations.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor. Description: Based on record review and interview, it was determined the vendor did not obtain all information for child 2 and 3.
Evidence:
1. Both Child 2 and Child 3 started within the last week and half. The Vendor stated she had name, phone number for the parents but did not have sick policy, parent employment information, and written authorization for emergency medical care included in her paperwork.
