Comments:
Two licensing inspectors conducted an unannounced, on-site monitoring inspection, which was initiated and completed on March 19, 2025. The on-site inspection began at 10:00 a.m. and ended at 3:00 p.m. The inspector reviewed compliance in the areas listed above. There were 108 children present and 21 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 27, 2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-A
Description: The center is required to have employees undergo a background check every five years. The most recent fingerprint check for staff #4 was five days late and the most recent central registry result was dated 10/21/19, making it more than five months overdue at the time of inspection.

Standard #: 8VAC20-780-130-A
Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1, who had been attending for three weeks and child #3, who had been attending for 18 days, did not have immunizations on file prior to attending.

Plan of Correction: Immunizations will be required before start date of children + submitted for approval before child begins

Standard #: 8VAC20-780-130-E
Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The most recent documentation of immunizations for Child #6, who recently turned two years, was dated 6/6/24.

Standard #: 8VAC20-780-500-B
Description: Repeat Violation
The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. Staff #1 did not clean and sanitize the diaper changing surface after changing a diaper.

Standard #: 8VAC20-780-550-E
Description: Shelter in place procedures shall be practiced a minimum of twice per year. There was only one shelter in place practiced in 2024.

Standard #: 8VAC20-780-560-J
Description: Tables shall be sanitized before and after each use for feeding. 1. Tables in the 2-year-old class and toddler class were not sanitized before use. 2. Children in the 2-year-old class were observed putting their shoed feet on the table while waiting for food to be served. Tables were not sanitized after they were instructed to get their feet off the table and food was then served.
