An unannounced, on-site (monitoring/renewal/focused) inspection was initiated and completed on 3/21/2025, as a part of the licensure period. The on-site inspection began at 8:50am and ended at 9:50am. The inspector reviewed compliance in the areas listed above. There were 36 children present and 6 staff. The inspector reviewed 4 children?s records and 4 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/28/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-130-E Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
No documentation of Child #2's immunizations every six months. Last immunizations documented from 18 months check up.Plan of Correction: Mom got copy of current shot record. Shot had been given 1/17/25.
Standard #: 8VAC20-780-160-A Description: Each staff member shall submit documentation of a negative tuberculosis screening completed within the last 30 calendar days of the date of employment.
Staff #1's negative tuberculosis screening was completed over three months prior to the date of employment.Plan of Correction: Will watch timeline of start date and TB tine test to ensure it's less that 30 days.
Standard #: 8VAC20-780-70 Description: Staff records shall contain employees address, and name, address and telephone number of a person to be notified in an emergency which shall be kept at the center.
