An unannounced ,on-site monitoring inspection was initiated on 3/14/2025 and completed on 3/14/2025. The on-site inspection began at 9:03 am and ended at 11:10 am. The inspector reviewed compliance in the areas listed above. There were 53 children present and 8 staff. The inspector reviewed 8 children's records on-site and 7 staff records electronically on 3/18/2025.

Standard #: 22.1-289.035-B-4 Description: Repeat Violation
The facility shall obtain a copy of the results of a criminal history record information check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Staff 1, who has been employed for more than three years, does not have a search of the child abuse and neglect registry and criminal history record information check from another state she has resided in within the past five years.Plan of Correction: Administrative staff stated that out-of-state background checks have been requested for staff 1 however, results are in the process of being located.
Standard #: 8VAC20-780-160-C Description: Repeat Violation
At least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section.
The TB screening for staff 1 and staff 2 was not updated every two-years.
*The most recent TB screening for staff 1 was completed 31 months ago.
*The most recent TB screening for staff 2 was completed 39 months ago.Plan of Correction: Administrative staff stated that staff 1 is in the process of updating her TB screening. Staff 2 obtained a current TB screening on 3/19/2025.
