During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated and completed on 3/18/25. The on-site inspection began at 2:35pm and ended at 5:20pm. The inspector reviewed compliance in the areas listed above. There were 52 children present and eight staff. The inspector reviewed five children?s records on-site and three staff records at the administrative building on 3/20/25. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/1/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-160-A
Description: Repeat Violation
The Tuberculosis (TB) documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
The TB documentation for Staff #1 (employed for two months) was not completed within thirty (30) days of employment, which is required.

Description: A tuberculosis (TB) screening is required every two years from the last screening.
The record for Staff #2 shows that their TB documentation was expired on the day of the inspection.

Description: The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
A report was not made to the department for a medical occurrence that required a child to receive outside medical treatment on 2/6/25.

Description: Repeat Violation
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
In a classroom that children use, there was multipurpose cleaner and two disinfectant sprays stored in an unlocked cabinet. Children were observed in this classroom during the inspection.

Description: Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.
Two groups of school age children were observed eating an afternoon meal. However, the children did not wash their hands or use disposable wipes before eating the meal.

Description: Repeat Violation
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all the required information.
Four injury reports reviewed were missing any future action to prevent recurrence of the injury, which is required.
