During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Technical assistance was provided and standards reviewed in the following areas:
8VAC20-780-270. Building maintenance.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 3.19.25. The on-site inspection began at 10:55am and ended at 1:35pm. The inspector reviewed compliance in the areas listed above. There were 19 children present and six staff. The inspector reviewed eight children?s records and two staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/1/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-150-B
Description: Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department.
The immunization record for Child #2 was not signed by a physician, his designee, or an official of the local health department.

Plan of Correction: All immunization records will be reviewed
thoroughly.

Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all the required information.
The record for Child #1 did not contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. The record for Child #2 did not the contain name, address, and phone number of one of the required designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Going forward, if a parent or family
state they do not have emergency
contacts other than the parent(s) or
guardian(s), we will no longer allow
them to enroll.

Standard #: 8VAC20-780-280-B
Description: Repeat Violation
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
In a three-year-old classroom, there were three sanitizing sprays in an unlocked cabinet. In a four-year-old classroom, there were two multi-purpose cleaners and a sanitizing spray in an unlocked cabinet. All are required to be locked.

Plan of Correction: All teachers have been instructed to
no longer keep cleaners in their room
cabinets due to the cabinets locking
features. All cleaners must be kept in
the janitors closet.

Standard #: 8VAC20-780-290-A-3
Description: In areas used by children of preschool age or younger, electrical outlets shall have protective covers.
There were three electrical outlet protective covers missing in the library and one missing in a four-year-old classroom. Children were observed in both areas of the center.

Plan of Correction: Outlet covers have been replaced and
teachers have been notified to return
the outlet cover after charging their
assigned computers and tablets.

Standard #: 8VAC20-780-440-E
Description: There shall be at least 12 inches of space between occupied cots, beds, and rest mats.
Occupied cots in a four-year-old classroom were not spaced at least 12 inches apart, which is required.

Plan of Correction: Staff has been advised to follow this
guideline.
