During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site focused inspection was initiated on March 24, 2025, completed on March 24, 2025, as a part of the provisional/licensure period. The on-site inspection began at 9:30 a.m. and ended at 11:00 a.m. The inspector reviewed compliance in the areas listed above. There were 71 children present and 22 staff. The inspector reviewed four children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 1, 2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-270-A
Description: Areas and equipment of the center shall be maintained in a clean, safe and operable condition.
In Classroom #4, there was visible dirt on the bottom counter cabinets, on the countertop, on the top of the half shelves and on the walls including the wall next to the changing table.

Plan of Correction: The area was cleaned deeper once it was safe to do so, without the children present. The wall near the changing table will be re-painted. The work order was placed.

Standard #: 8VAC20-780-290-A-3
Description: In areas used by preschool age or younger children, electrical outlets shall have protective covers.
In Classroom #2, there was one uncovered outlet within reach of the children. In Classroom #3, there was one uncovered outlet by an unoccupied crib and one uncovered outlet by an occupied crib and within reach of the child. In Classroom #4, there was one uncovered outlet within reach of the children.

Plan of Correction: Management immediately re inputted all outlet covers and met with the cleaning company to address replacing them when they are done cleaning in the rooms at night.

Standard #: 8VAC20-780-440-I
Description: Cribs shall be placed where objects outside of the crib are not within reach of the infants or toddlers.
In Classroom #3, there was a sound machine that was on and in use plugged into an outlet next to an occupied crib. The sound machine and cord were within reach of the child.

Plan of Correction: Corrected on site - immediately removed the sound machine from the proximity.

Standard #: 8VAC20-780-500-A
Description: Children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals and snacks.
Two children were observed eating breakfast in Classroom #1. After eating the two children joined the group for circle time and did not wash their hands.

Plan of Correction: Corrected on site - the children did return to wash their hands. Consistency and repetition will be demonstrated by the teacher, this is a part of the daily routine.

Standard #: 8VAC20-780-560-G
Description: When food is brought from home the food container must be clearly dated and labeled in a way that identifies the owner.
In Classroom #2, there were two bottles that contained food from home, without a date. In Classroom #3, there were four bottles that contained food from home, without a date and one sippy cup that contained food from home without a name or a date.

Plan of Correction: All items have been labeled with name and date. The teachers were mentored on the expectation to complete if the parent/guardian does not. Families were reminded to bring items labeled properly.
