Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site monitoring inspection was initiated on 03/24/2025 and completed on 03/24/2025. The on-site inspection began at 10:13am and ended at 1:08pm. The inspector reviewed compliance in the areas listed above. There were 61 children present and 11 staff. The inspector reviewed six children?s records and six staff records on-site. This inspection included document review, and tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/03/2025. A POC submitted after this date will not appear on the public website.

Description:	The center must request a search of the central registry prior to the employee's first day of employment. Staff #3 had a central registry search requested one day after their date of employment.

Description:	Repeat Violation: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #2, who had been enrolled for over three months.

Description:	Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #5 TB was dated three months prior to date of employment and has been employed for one month.

Description:	The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. On the Toddler/Pre-K Playground, paint was peeling and chipping off from the swing set where children were playing.

Description:	Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. In the older infant room, the foot-operated trashcan was not working, requiring staff to touch the exterior surface of the trash can.
