Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site monitoring inspection was completed on 03/25/2025. The on-site inspection began at 10:50 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were 72 children present and 16 staff. The inspector reviewed 7 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/03/2025. A POC submitted after this date will not appear on the public website.

Violations:

Standard #:	8VAC20-780-270-A
Description:	Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include breakage of any equipment. A broken cozy coupe car was observed on the playground. The steering wheel was off the car on the playground.
Plan of Correction:	The steering wheel and car have been removed from the playground. Staff have been reminded to trash broken toys ASAP.
Standard #:	8VAC20-780-350-A
Description:	The maximum group size limitations specified in Table 1 shall be followed whenever children are in care. 1. Birth up to 16 months -12 Children 2. 16 months up to 24 months -15 Children 3. 2 year olds - 24 Children 4. 3 year olds up to school age eligible -30 Children The group size for classroom #5 was over by 4 children.
Plan of Correction:	Three children from the toddler room shall be transitioned into the 2 y/o classroom. One child is moving. That leaves group size at 15.
