During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on March 24, 2025, and completed on March 24, 2025, as a part of the licensure period. The on-site inspection began at 9:02 am and ended at 12:20 pm. The inspector reviewed compliance in the areas listed above. There were 20 children present and six staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-350-C Description: When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group. On March 24, 2025, Staff #6 was observed supervising 7 children. The youngest child in the group was 23 months old. The required ratio for children ages 16 to 24 month is one staff per five children. Plan of Correction: Per the director: I will have staff come in 30 minutes earlier to cover.
Standard #: 8VAC20-780-510-P Description: When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Medication for child #6 was onsite, but the medication authorization had expired over 30 days ago. The medication had not been administered past the expiration of the authorization. The authorization had not been renewed, and the medication was not returned or disposed of as required. Plan of Correction: Per the director: Medication authorization form will be updated by the parent.
