During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection and investigation was initiated on 9/23/2024 and completed on 4/1/2025 with local agencies, in response to a complaint received by the licensing office on 9/10/2024 relating to allegations of Staffing And Supervision and Special Care Provisions. The inspector reviewed 1 child?s records and 2 staff records electronically on 4/1/2025. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Programs and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/9/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-370-1 Complaint related: No Description: When an infant is placed in his crib, he shall be placed on his back. On 9/9/2024 Staff #1 placed Child #2 in a crib, flipped him onto his stomach and walked away from the crib. Later, another unidentified child was placed in a crib on their stomach. Staff #1 proceeded to pat the child?s back.
Resting or sleeping infants shall be individually checked every 15-20 minutes. On 9/9/2024 no staff checked on sleeping Child #1 for over two hours.Plan of Correction: We are retraining all staff on this policy (Handwashing) at our next Staff Meeting on 4/16/25. Over the course of the next 30 days, management team will conduct daily diaper observations to ensure proper handwashing is happening during diaper changes. After 30 days, management will conduct random diaper observations throughout the week.
Standard #: 8VAC20-780-500-A Complaint related: No Description: Staff shall wash their hands with soap and running water before and after a diaper change and after contact with bodily fluids. On 9/9/2024 Staff #1 did not wash her hands after changing the diaper of Child #1. Staff # 2 did not wash her hands after changing a diaper prior to handing a pacifier to another infant in care. Five times multiple staff were observed wiping the noses of multiple children in care and lifting the lid of a trash can without washing their hands before interacting with other children. Plan of Correction: We are retraining all staff on this policy (Naptime Procedure) at our next Staff Meeting on 4/16/25. Over the course of the next 30 days, management team will conduct daily nap time checks to ensure nap checks are happening. After 30 days, management will conduct random nap checks throughout the week.
