Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site renewal inspection was initiated on 03/26/2025 and completed on 04/02/2025 as a part of the licensure period. The Department issued a NOTICE OF INTENT TO PLACE LICENSEE ON PROBATION AND REQUIRE THE LICENSEE TO CONTACT PARENTS, GUARDIANS, OR OTHER RESPONSIBLE PERSONS REGARDING HEALTH AND SAFETY VIOLATIONS on February 2, 2024 as a result of high-risk violations. The on-site inspection began at 10:05 AM and ended at 12:15 PM. The inspector reviewed compliance in the areas listed above. There were 57 children present and 13 staff. The inspector reviewed 5 children?s records and 3 staff records on-site and additional training documentation electronically on 4/1/25. This inspection included: document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/9/25. A POC submitted after this date will not appear on the public website.

Violations:

Standard #:	22.1-289.035-B-2
Description:	Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff B did not have a completed national criminal background check on file prior to working at the center.
Plan of Correction:	Newly hired candidates will wait to begin training only after fingerprint results have been received.
Standard #:	8VAC20-780-130-E
Description:	The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. One child record did not contain updated immunizations every six months until the age of two years. Child #4 had no updated immunizations on file.
Plan of Correction:	Parents will be reminded of submitting updated vaccinations very 6 months via the personal messaging app.

Description:	The center shall develop written procedures for injury prevention. An injury prevention plan could not be located during the inspection.
Plan of Correction:	The injury prevention plan will be completed on a monthly and yearly basis using the statistics from accident/injury reports
Standard #:	8VAC20-780-350-Q
Description:	Repeat Violation With a parent's written permission and a written assessment by the program director and program leader, a center may choose to assign a child to a different age group. Two child records did not include required documentation of assessments by the director to assign them to a different age group. (Children #3. 4)
Plan of Correction:	All children will be assessed with both the current and following age group assessment tools prior to transitioning into the next age group.
Standard #:	8VAC20-780-510-I
Description:	The center may administer prescription medication that would normally be administered by a parent to a child provided those drugs that were maintained in the original, labeled container. One prescription medication at the center did not have the prescription label attached to the box. (Child #6)
Plan of Correction:	All prescription medications will be stored only with proof of attached label on the container.
