During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced inspection was initiated on 3/10/2025 and completed on 4/2/2025 with a local agency in response to a complaint received by the licensing office on 3/6/2025 relating to allegations of physical plant and a serious injury. The inspector reviewed 1 child?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, additional violations that were not reported were found related to staffing and hazardous materials and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/15/2025. A POC submitted after this date will not appear on the public website.

The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

On 3/5/2025, Child #1 sustained a head injury and fractured hand while under the supervision of the center. The center was made aware that same day that Child #1 was taken to the hospital and did not submit a serious injury report to the Department.

Complaint Related

Areas and equipment inside the center shall be maintained in a clean, safe and operable condition.

1. On 3/5/2025, a large cabinet door in the Twos room, measuring approximately eight feet in height and 20 inches in width, fell on Child #1 (Age Two) while sitting at a table.
2. During onsite inspections on 3/10/2025 an 3/31/2025, the middle cabinet door that fell had been reattached with three hinges. However, the door is not fully operable as it cannot open completely without first opening the adjacent door.
3. During an onsite inspection on 3/10/2025, there was a nail protruding from the front of the same middle cabinet door, approximately three feet from the ground, that could entangle clothing or snag skin. During a follow up inspection on 3/31/2025, a sharp nail was protruding at the very bottle of the front of the middle cabinet door, that also could entangle clothing or snag skin. There was a child napping on a cot directly up against this cabinet door during both dates.
4. During an onsite inspection on 3/10/2025 and 3/31/2025, a piece of wood trim on the front of the middle cabinet door was broken at the bottom exposing jagged and splintered wood. A child was napping on a cot directly up against the cabinet door during both dates.

Description: Repeat Violation

Hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children.

A mop bucket full of cleaning solution and water along with one box containing multiple bottles of bleach and one box containing multiple bottles of a disinfectant solution was observed sitting inside the kitchen with the door wide open and easily accessible to children walking by.

Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 2/25/2025, Staff #1 notified Staff #4 the large cabinet door in the Twos room was broken and connected by only one hinge which allowed the door to freely swing side to side. On 3/3/2025, Staff #4 put in a work order for the door to be repaired. On 3/5/2025, Staff #2 was opening the door to obtain diapering supplies when the hinge came off and the door fell onto Child #1 (Age Two). Child #1 sustained a head injury and a fractured hand. The door narrowly missed falling on another child sitting at the table and a child that had just walked by the door.

Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications.

On 3/5/2025, Staff #1, Staff #2, and Staff #3 were the only staff present in the Twos classroom and none of the staff have documentation of lead teacher qualifications. The Twos classroom has not had a lead teacher for at least one month.
