Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated on 04/02/2025 and completed on 04/02/2025. The on-site inspection began at 10:33 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were 16 children present and 3 staff. The inspector reviewed 4 children's records and 3 staff records on-site. The inspection included: *Document Review *Tour of the Facility *Interviews *Observations Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	22.1-289.035-A
Description:	All applicants employees and applicants shall undergo a background check in accordance with subsection B every five years thereafter. The central registry check for staff 1 and staff 2 have not been updated every five years. Both expired 8 days ago.
Plan of Correction:	The program director stated that updated central registry checks will be requested today.
Standard #:	8VAC20-780-130-E
Description:	The center shall obtain documentation of additional immunizations once between each child's fourth and sixth birthdays. The immunization record for child 1, who is under the age of two years, has not been updated once every six months.
Plan of Correction:	The program director stated that the parent of child 1 will be asked to provide a current immunization record.
Standard #:	8VAC20-780-530-A
Description:	At least one staff in each classroom or area where children are present shall have, within 90 days of October 13, 2021: Current certification in CPR and First Aid. Staff 3, who was alone with a group of children, does not have CPR and First Aid certification.
Plan of Correction:	The program director stated that a first aid & CPR class will be scheduled for staff 3. Staff 3 normally has another teacher with her that has these certifications however, this person was unexpectedly absent today. Until staff 3 obtains first aid and CPR schedules will be adjusted to ensure that each group has a staff present with current CPR and first aid certification.
