Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 3/31/2025, as a part of the licensure period. The on-site inspection began at 10:50am and ended at 2:20pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-780-130-A
Description:	Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. The physical examination in the record for Child #5 was one week overdue.
Plan of Correction:	The physical was obtained and placed in the child record. Physicals will be obtained within required time frames.
Standard #:	8VAC20-780-60-A
Description:	The center is required to maintain a record for each child enrolled which contains all required information. The record for Child #2 did not contain documentation of the child's first date of attendance. The record for Child #5 did not contain the addresses of two designated people to call in an emergency if a parent cannot be reached
Plan of Correction:	The center will contact the parents for the information.
Standard #:	8VAC20-780-270-A
Description:	The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The corner of the wall at the entrance of the Stem Lab was damaged creating sharp edges towards the bottom of the wall, where children play.
Plan of Correction:	The area was temporarily covered during the inspection and a work order was submitted.
Standard #:	8VAC20-780-420-E-3
Description:	Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of child #4 had not been updated in over 3 years.
Plan of Correction:	The center will request that the parent review the child's information in the file.
