Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 4/1/2025. The on-site inspection began at 8:55 am and ended at 11:00 am. The inspector reviewed compliance in the areas listed above. There were 28 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/10/2025. A POC submitted after this date will not appear on the public website.

Standard #:	22.1-289.035-B-1
Description:	The center is required to obtain a completed sworn statement prior to the employee's first day of employment. The sworn statement for Staff #3, who is currently working, was dated 21 days after their first day of employment.

Description:	Providers must obtain a completed national criminal background check prior to the employee's first day of employment. The criminal background check for Staff #3, who is currently working, was dated 9 days after their first day of employment.

Standard #:	8VAC20-780-130-A
Description:	The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #3, who had been attending for seven months, did not have documentation of an immunization record on file.

Standard #:	8VAC20-780-40-M
Description:	The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. There was no list in one of the rooms where there was a child in attendance with a known food sensitivity.
