22.1 BACKGROUND CHECKS CODE; CARBON MONOXIDE
During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was completed on April 2, 2025. The on-site inspection began at 11:07 am and ended at 2:28 pm. The inspector reviewed compliance in the areas listed above. There were 44 children present and 12 staff including administrative staff. The inspector reviewed 10 children's records and 5 staff records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 11, 2025 . A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-3 Description: The center is required to obtain a copy of the results of a search of the central registry.
Staff#4, who had been employed for 3 months did not have central registry results. Staff#4 was working alone with children.Plan of Correction: Staff will not work alone in a classroom until all required background check results are received. Additionally, management will track & monitor all background checks monthly for expiration dates and follow up on any that remain pending.
Standard #: 8VAC20-780-160-C Description: A tuberculosis(TB) screening is required every two years from the last screening. Staff#1 updated screening was due 27 days ago. Plan of Correction: All new hires will provide TB test results in pre-onboarding meeting scheduled prior to their employment start date. Management will monitor files monthly to ensure renewal of any expired documents occur timely.
Standard #: 8VAC20-780-60-A-8 Description: A written care plan is required for each child with a diagnosed food allergy, to include instructions from physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child#8 who has been enrolled for 3 days , has a diagnosed food allergy and did not have a written plan in their file. Plan of Correction: All families are required to provide all enrollment paperwork, including allergy/food sensitivity documentation during paperwork appointments scheduled prior to their start date. Management will review all paperwork to ensure compliance.
Standard #: 8VAC20-780-280-B Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Multiple containers of foam hand soap and one bottle of sanitizer with the warning labels, "keep out of reach of children," were unlocked in the infant room closet.Plan of Correction: Teachers have been retrained on proper storage of chemicals. Safety captain and management will monitor all classrooms daily to ensure chemicals are stored properly.
