Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 04/03/2025. The onsite inspection began at 11:35 am and ended at 12:35 pm; the staff records were reviewed on the same day at the administration office. The inspector reviewed compliance in the areas listed above. There were 33 children and 5 staff. The inspector reviewed 5 children?s records and 5 staff records.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: A tuberculosis (TB) screening is required every two years from the last screening.

The TB screening for Staff 3 was overdue by 6 months.

Plan of Correction: The staff was notified to get the screening updated.

Standard #: 8VAC20-780-510-P
Description: When an authorization for medication expires, the facility shall notify the parents that the medication must be picked up within 14 days of the authorization expiring or a new authorization should be completed.

The authorization for Medication 1 was expired by 1 month and the medication was administered once during the expired period.

Plan of Correction: Staff stated they would get the authorization updated.
