Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced monitoring inspection was initiated and completed on 4-2-25. The Department issued a Notice of Intent to place the licensee on probation for a period of six months; and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations on 3/12/25 as a result of high-risk violations. The on-site inspection began at 10:30am and ended at 1:45pm. The inspector reviewed 2 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/14/25. A POC submitted after this date will not appear on the public website.

Standard #:	8VAC20-780-80-A
Description:	The Center is required to keep a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs. During inspection in the Infant A classroom, there were 3 children present, but the written record reflected there were 6 children present.
Plan of Correction:	We have met with all teachers to ensure understanding on the regulation regarding written attendance. We have also implements thrice daily manager checks of all roll call sheets to ensure compliance.
Standard #:	8VAC20-780-280-B
Description:	(Repeat Violation) The Center is required to keep hazardous substances in a locked place using a safe locking method that prevents access by children. During inspection in the staff bathroom directly off the hallway and where children have access, 1 container of glass cleaner containing a label that stated ?Keep out of Reach of Children? and the word ?Caution? were stored in an unlocked area.
Plan of Correction:	We have met with the housekeeper to ensure her understanding of the regulation regarding all chemicals must be locked. We have also implemented daily compliance checks with all directors and team leaders walking the building to ensure compliance.
