During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated and completed on 4-2-25 in response to a self-report received by the licensing office on 3/27/25 relating to Supervision. The Department issued a Notice of Intent to place the licensee on probation for a period of six months; and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations on 3/12/25 as a result of high-risk violations. The inspector reviewed 2 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to sight and sound supervision and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/15/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-F Description: (Repeat Violation) The Center is required to ensure that children under 10 years of age always be within actual sight and sound supervision of staff.
On 3/11/25, in the Creme Prep CD classroom, 8 children were observed playing in 2 different areas of the classroom which is separated by a half wall. Staff #1 was the only staff member present and was observed moving back and forth between the 2 areas leaving some of the children alone when she would leave each area.Plan of Correction: When this self reported incident was investigated, the licensor went back to review camera footage that was prior to retraining our team on 3/27/2025. Our training was conducted to comply with our previous licensing visit concerning proper supervision in classrooms that have a half wall. The training clarified what proper supervision looks like and how to use additional resources that we have provided, i.e.walkie talkies.
