During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection was initiated and completed on 4/3/2025 in response to a complaint received by the licensing office on 4/1/2025 relating to allegations of diapering, care/protection/guidance, and behavioral guidance. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, violations that were not reported were found related to record keeping, physical plant, handwashing and over-the-counter skin products and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/16/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-80-A Complaint related: No Description: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
In the Preschool 1A classroom there were six children present and 5 were signed in on the tablet and 8 were signed in on the paper attendance sheet.Plan of Correction: The staff immediately wrote the children who were not present on the Child Transition Sheet and updated the digital record.

The toilet tank lid was missing from the toilet used by children located in the Preschool 1A bathroom.Plan of Correction: The Center Director immediately covered the top of the toilet safely and indicated "out of order".

When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 3/26/2025, there were standard size golf balls located in the Preschool 1A classroom which is considered a choking hazard. Child #1 put one of the golf balls in their mouth for approximately twelve seconds before gagging and spitting the ball out. Staff in the room were not aware that Child #1 had put a golf ball in their mouth.Plan of Correction: Teacher removed golf ball from the area after the Center Director was notified that they had placed a golf ball in their mouth. All toys were examined by Center Director.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
On 4/2/2025, Child #1 (Age 2) and Child #2 (Age 2) were wrestling on the floor before continuing to wrestle on top of a table approximately 15.5 inches in height. Both children fell from the table and Child #1 was visibly upset and crying. Staff #1 was the teacher in charge of the classroom at the time of incident and did not provide any behavioral guidance or redirection to encourage appropriate behavior and resolving conflicts.Plan of Correction: Staff reviewed Educational Policies and Procedures with Center Director. Staff shall receive and complete "GUI106: Foundations of Positive Guidance" administered by Childcare Educational Institute by no later than 4/18/2025.

Children's hands shall be washed with soap and running water before and after eating meals or snacks. Staff's hands shall be washed with soap and running water before and after diaper changes and helping children use the toilet.
Staff #1 did not wash hands between diaper changes and helping five children in the Preschool 1A room use the toilet. Five children did not wash hands after finishing breakfast and beginning another activity.Plan of Correction: Handwashing procedure was reviewed by staff, as well as current posted handwashing procedure posted in classroom.

Children younger than three years old shall have diapers changed on a nonabsorbent surface for diapering, such as a changing table or countertop designated for changing. Disposable diapers shall be disposed in a receptacle that is either foot-operated or used in a way that the staff member's hand does not touch the exterior surface during disposal.
Two children, both two years old, had their diaper/pull up changed by Staff #1 while standing up in the bathroom. Diapers were disposed of in a swing top trash can in the bathroom and Staff #1's hand touched the exterior surface during disposal of the diapers.Plan of Correction: The Center Director reviewed the diaper changing procedure with the staff member, and monitored each step as staff completed multiple times. The diaper changing receptacle was replaced immediately to a handsfree container with foot-pedal.

In the Preschool 1A classroom, there were six children present, ranging in age from two to three years old, and five sunscreens were stored in an unlocked cabinet under the changing table.Plan of Correction: A new lock was immediately placed on the diaper changing station by the Center Director.

In the Preschool 1A classroom, there were six children present, ranging in ages from two to three years, and there were two diaper creams stored in an unlocked cabinet beneath the changing table.Plan of Correction: A new lock was immediately placed on the diaper changing station by the Center Director.
