An unannounced, on-site monitoring inspection was initiated and completed on April 9, 2025. The on-site inspection began at 10:00am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 1 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard #: 22.1-289.035-B-1 Description: The center is required to obtain a completed sworn statement prior to the employee's first day of employment.
The sworn statement for staff #1, was dated ten weeks after their first day of employment.Plan of Correction: The sworn statement for staff #1 was completed today.
Standard #: 8VAC20-780-160-C Description: A tuberculosis (TB) screening is required every two years from the last screening.
Staff #2s updated screening was due two days ago.Plan of Correction: Staff #2 has scheduled a TB test for today, April 9, 2025. The results will be in on April 11, 2025.
