8VAC20-780 Special Care Provisions and Emergencies
During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation

An unannounced inspection was initiated and completed on 4/4/2025 in response to a complaint received by the licensing office on 4/2/2025 relating to allegations of Procedures for Emergencies. This inspection included document review, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/16/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-550-O Complaint related: Yes Description: The center shall notify the parent by the end of the day of any known minor injuries.
On 3/31/2025 Child A sustained a minor injury on their back. Staff was aware of the incident but did not notify the child?s parent by the end of the day.Plan of Correction: On 4/7/2025, the staff member was given training on Communicating with Parents, Staff and Children. The staff member was also given COA training on 4/7/2025 on Licensing. Moving forward, all staff will ensure to communicate with management of a child injury, so it can be reported to the parent.
Standard #: 8VAC20-780-550-P Complaint related: Yes Description: The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: Date and time when parents were notified.
1. On 3/31/2025 Child A sustained a minor injury on their back and the written record of child's injury was not documented on the same day.
2. The written injury report that was documented the next day, did not have the date and time the parent was notified once it was completed.Plan of Correction: On 4/7/2025, the staff member was given training on Communicating with Parents, Staff and Children. The staff member was also given COA training on 4/7/2025 on Licensing. Moving forward, all staff will ensure to communicate with management of a child injury, so it can be reported to the parent. Incidents and accidents must also be documented on the same day of the incident.
