During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on April 2, 2025, and completed on April 2, 2025, as a part of the licensure period. The on-site inspection began at 9:20 am and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. There were 77 children present and 13 staff. The inspector reviewed eight children?s records and eight staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 25, 2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-130-E
Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
1. The immunization record for child #6, age 2 and a half years and who has been enrolled for two years, had not been updated since enrollment.
2. The immunization record for child #8, age 2 and a half years and who has been enrolled for two years, had not been updated since enrollment.

Plan of Correction: Per the director: Administration has requested that home office apply a macro to child file audit spreadsheet that will inform administration when the 6 month mark is approaching so that a request to update immunization record can be issued. Parents will be informed that child cannot return to care unless their records are appropriately up to date.

Standard #: 8VAC20-780-140-A
Description: Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance. Child #4, enrolled for seven weeks, did not have documentation of a physical on file.

Plan of Correction: Per the director: Parent was immediately informed and form given to them. Parent was told that child would not be able to return to care until it was completed and turned in to us. Form was returned 4/05/2025. Parents will be asked to provide their child's physical prior to child's start date.

Standard #: 8VAC20-780-270-A
Description: Repeat
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Peeling paint was observed around the door frame in the school age classroom. The paint chipped off at the touch.

Plan of Correction: Per the director: Maintenance was informed immediately and area was repaired. Company will implement an app for accountability with classroom safety checklist.

Standard #: 8VAC20-780-340-F
Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff. Child # 9, eight years in age, was observed walking out of one building unattended, across the blacktop area and into the second building. Administration stated the child was going to see a family member who works at the center. Child #9 was out of sight and sound supervision of staff.

Plan of Correction: Per the director: Sight and sound rule was reiterated to child and her aunt, who is a staff member here. All other children in the school age program were reminded of the policy and Tuckaway's expectations.

Standard #: 8VAC20-780-400-B
Description: Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts. Just before naptime, staff #9 was observed yelling ?Good night?, ?good night? in a harsh tone towards a child who was trying to ask the staff a question. Staff #9 leaned down to the child?s face when repeatedly yelling ?goodnight?. Administration acknowledged hearing staff #9 use the harsh tone.

Plan of Correction: Per the director: Teacher received training by watching webinar entitled "Frustrating Temperament Traits: Strengths- Based Approaches to ensure Young Children Thrive" training was completed the day following inspection, 4/03/2025. Administration discussed strategies to mitigate stress in the classroom and support. Tuckaway policies for behavior guidance reviewed.

Standard #: 8VAC20-780-500-A
Description: Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. One child in the toddler room was observed getting up from snack and going to play without having his hand washed.

Plan of Correction: Per the director: Teachers were reminded of this requirement and directed to assist the children in handwashing before and after meals.

Standard #: 8VAC20-780-550-E
Description: Shelter in place procedures shall be practiced a minimum of twice per year. There was no documentation of a second practice shelter-in-place drill for the 2024 year.

Plan of Correction: Per the director: Going forward, shelter in place will be performed in the months of April and October.

Standard #: 8VAC20-780-570-E
Description: Prepared infant formula shall be refrigerated, dated and labeled with the child's name. One of two infant bottles was not dated in the infant room.

Plan of Correction: Per the director: Teachers will inspect each bottle as they are stored in refrigerator. Specific parent was notified of this lapse. All parents were reminded of this requirement. Tape and marker provided in classroom.
