Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Technical Assistance:

The following standards were reviewed: 550.I - posting of emergency phone numbers Requirement of annual training for all staff who may work in a caregiver/service role. Refer to definition of staff - 8VAC20-780.10 Ceiling vent fans in bathroom of classroom C & E need to be repaired/replaced. Standard 60.A.5 - requires two emergency contacts other than the parent. Standard 530.A.1 & 2 outlines accepted CPR & First Aid training for staff.

Comments:

An unannounced, on-site renewal inspection was initiated on 04/08/2025 and completed on 04/08/2025. The on-site inspection began at 10:10 am and ended at 12:10 pm. The inspector reviewed compliance in the areas listed above. There were 51 children present and 6 staff. The inspector reviewed 9 children's records on-site and 8 staff records electronically on 04/10/2025. The inspection included: *Document Review *Tour of the Facility *Interviews *Observations Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/22/2025. A POC submitted after this date will not appear on the public website.

Description:	Children's Records shall contain all of the required elements. 1. The enrollment record for child 1 and child 2 lacked a second emergency contact. The address for the first emergency contact was not included.
