An unannounced inspection was initiated on 4/8/25 and completed on 4/14/25 in response to a complaint received by the licensing office on 3/27/25 relating to allegations of supervision and records. The inspector reviewed 1 child?s record and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-50-A Complaint related: Yes Description: Children's records shall be made available to parents on request.
1. On 3/27/25, Child # 1's parent requested a copy of the child's record to include a copy of all signed documents and the health forms.
2. The parent stated that the center only provided them a copy of the health forms, and refused to provide a copy of all other signed documents.
3. Staff # 1 stated that they thought that documents other that the health forms were for the center only, and thought they did not have to release that information upon request.Plan of Correction: Children's records will be made available on parent's request.
