During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated and completed on 4/3/2025, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 42 children present and 12 staff. The inspector reviewed 5 children?s records and 5 staff records electronically on 4/13/2025. This inspection included document review,
tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/21/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-3 Description: The center must request a search of the central registry prior to the employee's first day of employment. Staff #1 and #2 did not have documented dates of requested central registry background prior to employment. Plan of Correction: Central Registry results were added to Staff 2?s file. Staff 1 no longer works with the company. Moving forward, Center Director will ensure this is requested and printed prior to the employee?s first day of employment.
Standard #: 8VAC20-780-260-A Description: The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. The most recent fire inspection report was dated 2023. Plan of Correction: Our annual Fire Inspection has been completed and is on file at the center.
Standard #: 8VAC20-780-280-B Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. In the two-year-old, on a low shelf inside an unlocked cabinet there were multiple spray bottles of chemicals within reach of children. Plan of Correction: All employees were retrained on the expectation that all potentially hazardous substances are to remain locked away from children at all times at the center?s Staff Meeting on 4/16/25. Center Management will conduct weekly observations of classrooms to ensure this is being followed.
Standard #: 8VAC20-780-500-A Description: Staff shall wash their hands with soap and running water before a diaper change. Staff in the infant room did not wash their hands before changing a diaper. Plan of Correction: All employees were retrained on the Handwashing Procedure at the center?s Staff Meeting on 4/16/25. Center Management will conduct weekly observations of classrooms to ensure this is being followed.
Standard #: 8VAC20-780-500-B Description: Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Staff in the infant room used her hand to lift the lid of the trash can to throw away a soiled diaper. Plan of Correction: All employees were retrained on the expectation that they are to properly dispose of diapers without touching the trash can lid at the center?s Staff Meeting on 4/16/25. Center Management will conduct weekly observations of classrooms to ensure this is being followed.
Standard #: 8VAC20-780-570-I Description: A one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care shall be maintained at the center. The center did not maintain a the required one-day?s emergency supply of formula. Plan of Correction: Enough emergency formula has been purchased to cover the number of children assigned to the room. Center Management will check dates periodically to ensure we order replacement formula before the next expiration date.
