During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on April 15, 2025. The on-site inspection began at 9:45am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 82 children present and 18 staff. The inspector reviewed ten children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable conditions. Unsafe conditions shall include, but not be limited to, chipped or peeling paint. There were several areas in classroom #1 with chipped and peeling paint.

Plan of Correction: We will cover the peeling paint areas with pictures or furniture until the wall is repaired and painted. We will submit a work order today.

Standard #: 8VAC20-780-290-A-3
Description: Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. There was a power strip laying on a shelf with several outlets uncovered within reach of children in classroom #2.

Plan of Correction: The electrical power strip was removed and teachers will be reminded to keep the protective covers on all electrical outlets when not in use.

Violation was corrected during the inspection.

Standard #: 8VAC20-780-500-B
Description: Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. The staff in classroom #3 repeatedly used their hands to lift the garage lid to dispose of soiled diapers, instead of using the foot pedal.

Plan of Correction: We will retrain staff on proper diapering procedures to include using the foot-pedal storage system that is provided and not using their hands to lift the lid when disposing of diapers
