During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Recommend follow up with DOH regarding potable water testing plan. Discussed printed/posted instructions for diaper changing table and solutions (sanitizer versus disinfectant).

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 04/18/2025, as a part of the licensure period. The on-site inspection began at 9:50am and ended at 11:50am. The inspector reviewed compliance in the areas listed above. There were 90 children present and 19 staff. The inspector reviewed 7 children?s records and 3 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/25/2025. A POC submitted after this date will not appear on the public website

Violations:
Standard #: 8VAC20-780-150-B
Description: Report immunizations received shall be signed by a physician, his designee, or an official of a local health department.

Plan of Correction: Parents have requested signed immunization record.

Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain name, home address, home phone number, work phone number and place of employment of each parent who has custody.

The record for child #2 did not contain name, home address, home phone number, work phone number and place of employment of their second parent, who has custody.

Plan of Correction: Parent completed emergency card and information and it has been added to their file.

Standard #: 8VAC20-780-260-B
Description: After the first license, annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for water supply and sewage disposal system.

An environmental sanitation report weas not available at inspection.

Plan of Correction: Director contacted local health department for a copy of inspection report and provided documentation from health department.

Description: Staff shall request at least annually parent confirmation that the required information in the child's record is up to date.

Child #4 and Child #5's records did not document parent confirmation that the required information in the child's record is up to date annually. Both children have been in care for over a year.

Plan of Correction: Requested file update from parents.

Standard #: 8VAC20-780-440-B
Description: Cots shall be identified for use by a specific child.

The cots in PreSchool classroom were not labeled in in a way to identify use by a specific child.

Plan of Correction: The identified classroom has labeled their cots.
