During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 04/14/2025, as a part of the licensure period. The on-site inspection began at 2:05 PM and ended at 4:00 PM. The inspector reviewed compliance in the areas listed above. There were 48 children present and 10 staff. The inspector reviewed 6 children?s records and 5 staff records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-280-B
Description: Repeat violation
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. The swinging doors to the kitchen in which are accessible hazardous substances do not have a means to make the kitchen inaccessible to the children when no one is in the kitchen.

Plan of Correction: The provider's response:
Sliding bolts were installed on the doors to the kitchen however they were installed on the wrong side of the doors. Maintenance was contacted during the inspection to return and install the bolts on the correct side of the doors to ensure that children cannot access the potentially hazardous substances in the kitchen..

Standard #: 8VAC20-780-510-P
Description: When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. The parent authorization for emergency medication for a child expired one month ago.

The parent was contact during the inspection to renew the medication authorization today.;
